Wisconsin Illinois Senior Housing Inc. (WISH) and Carriage Healthcare Co., Inc. (CHC) are proud to announce that two of our communities — Fair Oaks Health Care Center in Crystal Lake, IL, and Holton Manor in Elkhorn, WI — have earned the prestigious 2025 Bronze National Quality Award from the American Health Care Association and National Center for Assisted Living (AHCA/NCAL).
The Bronze National Quality Award recognizes senior care providers that demonstrate a strong commitment to improving the lives of residents through high-quality care, operational excellence, and continuous improvement. Applicants undergo a rigorous review based on the Baldrige Performance Excellence Program, where trained examiners evaluate organizational practices against nationally recognized standards.
At both Fair Oaks Health Care Center and Holton Manor, the journey toward the Bronze Award strengthened collaboration, clarified organizational goals, and reinforced each team’s shared commitment to exceptional care. Fair Oaks Administrator Noreen Zaio reflected on the process, stating, “The application process gave us a meaningful opportunity to evaluate what we do well and grow in other areas. We learned the power of data, communication, and shared purpose in advancing care.”
Fair Oaks’ achievement highlights its person-centered approach, focus on dignity, and ongoing dedication to improvement - values shared across all WISH and CHC communities.
Holton Manor’s team equally embraced the challenge with energy, purpose, and pride. Their award reflects the dedication and compassion their staff bring to residents and families every day, ensuring a warm, supportive environment built on excellence and trust.
To understand the significance of this achievement: out of roughly 16,000 nursing home providers in the U.S., only 486 earned the Bronze Quality Award in 2025.
Following these Bronze-level accomplishments, both facilities plan to pursue the Silver National Quality Award, which requires providers to demonstrate further measurable progress using systematic, data-driven performance improvement approaches.
