Long term care is for those who need permanent full-time care with daily activities like bathing, dressing, eating, moving from one place to another, hygiene, toileting, etc. Long term care is usually provided in a Skilled Nursing Facility (SNF) like Montello Care Center.
Skilled nursing care helps with medical needs including rehab therapies, medication management, IV therapy, colostomy care, and other services, memory care, wound care, medication management, pain management, and more. Care is administered by an RN.
Assisted living is offered right here on our campus, at Residences on Forest Lane. We offer private or shared suites and a higher level of assistance than independent senior living offers. Staff help with activities of daily living like hygiene, eating, moving, medication reminders, etc.
We offer both outpatient and short-term inpatient rehab therapy. Physical, occupational and speech therapies as well as other modalities are available. Many patients use physical therapy prior to surgery, or to help with post-surgery recovery. Therapy often occurs after a hospital stay, like after knee or hip surgery or following injury, accident or illness.
Transitional care is an important part of your care that coordinates the continuity of your healthcare during a move from one place to another, like from a hospital to a nursing/rehab center, and then back home. Its goal is to have caregivers in all settings work together to share information and ensure you have the best care and experience when moving from one care setting to another.
Yes, we have nurses either in our building or on-call 24-hours a day.
If falls are a concern, the care plan may include interventions and special precautions to help address this issue that will be followed by our team. Many of our daily programming and activities offer exercise classes that focus on strength and balance, which may assist in reducing the risk of falling.
Yes, our med management program is monitored by our professional licensed RNs. We help distribute medications based on your prescription from their doctor.
At admission, our nurses and care team work together to assess your current health, review your medical history and your personal needs to determine how we can best meet your needs. They will ask you about your preferences and your normal routines to develop a care plan just for you.
The cost to live at Montello Care Center varies so please call us. Our Business and Social Services offices will help you understand your options prior to move-in. We accept Medicare, Medicaid, HMOs, Medicare replacements, long term insurance and private pay, with some limitations.
We provide ongoing continued education and training, in-staff clinics, clinical care protocols, and multiple quality assurance measures to reinforce our strong standards and stay current with state and federal health regulations.
Staffing-to-resident ratio varies, depending on the number of residents we have & their needs. We staff based on number of patients and residents and the level of care of each care plan in place.
Creative activities, outings and events vary each week. They are determined by season and are designed by our activities staff to engage mind, body and spirit. In addition to in-house games, crafts, music, exercise classes and special events, we regularly plan fun activities out in the community such as lunch outings, scenic rides, shopping, fishing, or other engaging activities. There’s always something to look forward to!

Yes, we hold monthly resident council meetings to find out what residents want and what they'd like to include in our calendar.
Emphasizing choice is important in a dining program. In addition to the unique entrees featured at each of our 3 daily meals, we also offer an “always available” menu for residents. Snacks are available any time to residents. We have 3 dining rooms including one in our newer rehab wing. In-room dining is also available.

Yes, whether you come for a long term or short term rehab stay, you are welcome to bring items of comfort for your room. We do provide you with the basic furniture including a bed, nightstand, dresser, chair, etc. We always encourage residents to make their room as comfortable as possible.
Comfortable clothes, athletic shoes, pajamas, a robe, slippers, personal grooming items and anything you might want on a daily basis. Your family can do your laundry or we can do it for you. We have movies to watch and activities you can choose to participate in while you’re not in therapy, or you can bring your own entertainment. Wifi is available in the building.
We can provide local transportation to doctor's visits for an additional fee. Transportation is free to and from activities and outings.
We welcome visitors and work with families and friends to visit their loved ones at a time that works best for them. To honor quiet hours and our residents' privacy, we encourage visiting hours between 9 am and 8 pm. To keep the building secure, doors are locked at 8 pm, but entry is available via a doorbell.
We don’t allow pets to live at Montello Care Center, but pets can come to visit their owners. Occasionally, we have therapy pets that come in to visit with residents who are interested. Pet visits are always a highlight for staff and residents alike.
We work with each family to support their needs as best we can, including accommodating urgent admissions. Residents receive a full care assessment prior to moving in, which helps our care team develop an individualized care plan. In some cases, a new admission can occur in as little as a few hours.
