Following certain types of major medical events such as a brain injury or stroke, amputation, or other life-changing events, it may be necessary for you or a loved one to receive extra care and rehabilitation after leaving the hospital but before returning home.

Skilled nursing at Fair Oaks Health Care Center can help. We coordinate with the hospital to seamlessly transition you or your loved one into our care community and then back home. Our highly trained nursing staff work with physicians to complete a detailed assessment and develop a personalized care plan to meet individual needs and ensure smooth transitions.
