Nurses:
RNs & LPNs

A registered nurse (RN) administers medication and treatment to patients, coordinates patient care plans, performs diagnostic tests and analyzes results, instructs patients on how to manage illnesses, and oversees other workers such as LPNs, CNAs, and home care aides. To become an RN, one of the following is required: a Bachelor of Science in nursing (BSN), an associate's degree in nursing (ADN), or a diploma from an approved nursing program.

Licensed nurse practitioners (LPNs) provide basic medical and nursing care such as checking blood pressure and inserting catheters, ensuring comfort of patients, helping with daily functions, discussing health care with patients and reporting patient status to RNs and doctors. Becoming an LPN requires the completion of an accredited practical nursing program which takes about one year.

Physical and Occupational
Therapists (PT/OT):

With more people receiving care at home following hospital or nursing home stays, demand is high for physical and occupational therapists as well as their counterpart assistants (PTA & COTA). Our physical/occupational therapy assistants work hand-in-hand with our PTs and OTs in our patients’ homes carrying out the tailored care plans designed and customized for each patient. 

Speech
Therapists (ST):

Speech therapists assist with cognition deficits and swallowing issues and are an important part of Transitions’ home health care teams. We often have open positions for all types of therapists. Be sure to check out our job listings regularly.

Health Aides:

Are you looking for a new challenge or ready for an exciting career change? If you are caring, enjoy helping others, are respectful of others, and are fulfilled by improving the lives of others, becoming a home health aide (HHA) may be for you.

It's also a great way to start a nursing career. Home health aides help patients with limited health care needs as well as personal needs within the patient's home. Short training programs make this a fast way to start a career in health care. Once you've completed all course requirements you are then tested and can become certified.
