Nurses:
RNs & LPNs

A Registered Nurse (RN) administers medication, coordinates care plans, performs diagnostic tests, educates patients, and supervises other staff such as LPNs and CNAs. To become an RN, you need a Bachelor of Science in Nursing (BSN), an Associate's Degree in Nursing (ADN), or a diploma from an accredited program.

Licensed Practical Nurses (LPNs) provide basic care like checking vital signs, inserting catheters, and assisting with daily tasks. LPNs report patient status to RNs and doctors. Becoming an LPN requires completing an accredited practical nursing program, typically lasting one year.

Physical and Occupational
Therapists (PT/OT):

With more people receiving care at home, the demand for physical and occupational therapists, along with their assistants (PTAs & COTAs), is high. Our therapy assistants work closely with PTs and OTs, delivering personalized care plans in patients’ homes.

Speech
Therapists (ST):

Speech therapists assist with cognition deficits and swallowing issues and are an important part of Transitions’ home health care teams. We often have open positions for all types of therapists. Be sure to check out our job listings regularly.

Health Aides:

Are you looking for a new challenge or ready for an exciting career change? If you are caring, enjoy helping others, are respectful of others, and are fulfilled by improving the lives of others, becoming a home health aide (HHA) may be for you.

It's also a great way to start a nursing career. Home health aides help patients with limited health care needs as well as personal needs within the patient's home. Short training programs make this a fast way to start a career in health care. Once you've completed all course requirements you are then tested and can become certified.
